Mobility reimbursement program.

You can begin by looking for your new vehicle. We encourage you to visit your local Chrysler, JeepĀ® and Dodge dealerships or visit our vehicle web sites to determine which vehicle best suits your travel requirements.


Contact a reputable and qualified adaptive equipment installer to ensure that your chosen vehicle can be adapted to your specifications. You can find a list of National Mobility Equipment Dealers and factory-approved conversion specialists in your area by using DC website Locator.


Obtain a prescription or note from a licensed medical doctor on physician's letterhead for reimbursement of the adaptive equipment. (Some adaptations like hand controls and wheelchair or scooter hoists do not require a doctor's approval. Check with your dealer and adaptive equipment installer to determine if a prescription or note is needed for your adaptations.)


Fill out an Automobility Application form. Forms are available at any dealership, adaptive equipment installer or from the Application section of DC web site. (The application form on company web site is available in a printable, .PDF format.)


After your vehicle has been adapted, submit your application and required documentation for reimbursement. (You must get the required signatures before sending it in for your refund. Check the Application section of DC website for complete details.)
