Provide co-payment assistance for those who financially and medically qualify.

Funding avaliable for:
- breast, lung, prostate, kidney, colon, pancreatic and/or head/neck cancers
- malignant brain tumor
-lymphoma
- sarcoma
- diabetes
- multiple myeloma
- myelodsyplastic syndrome (and other pre-leukemia diseases)
-selected autoimmune disorders
-secondary issues as a result of chemotherapy treatment

PAF Co-Pay Relief (CPR), a patient assistance program, provides direct financial assistance to qualified patients; assisting them with medication co-payments their insurance requires. The program offers personal service to all patients through the use of CPR call counselors. The call counselors works directly with the provider of care to obtain necessary medical, insurance and income certification in an expeditious manner. Upon approval, payments are made; to the doctor, pharmacy, or the patient directly.

-The number of people living in their household
- What the entire household gross income for the previous year was

Following information of the doctor who is treating their disease:

We do look at income, but eligibility is not strictly based on that. We also take into consideration the cost of living where they are, how many people are in their household, and what they pay out in medical bills, doctor bills, and prescription drugs for the entire household.
