Nestle HealthCare Nutrition Patient Assistance Program

Does not offer assistance with BOOST products. The Nestle Health Care Nutrition Patient Assistance Program helps patients whose sole source of nutrition is the requested product and who lack financial resources or reimbursement options (for example insurance, government assistance or other programs) to cover product costs. The products offered under the program meet special medically determined nutrient requirements which cannot be achieved by modification of the normal diet alone and are generally used for tube feeding. The program provides a maximum of 12 cases of product per year. The amount approved is at the sole discretion of Nestle Health Care Nutrition.
