Hospital Discharge Planning, Nursing Home Admissions , and Social Work Departments
The Area Office on Aging of NWO, Inc., PASSPORT Preadmission Unit will be closed on
Monday, September 7th, 2026 in observation of Labor Day.
THERE WILL BE NO LOCAL COVERAGE PROVIDED ON MONDAY, September 7th,
2026. All requests submitted after midnight on Sunday, Sept. 6th, will be processed within
OAC timelines.
Please try to submit requests for anticipated discharges on Friday, Sept. 4th, by 3:00 pm
to allow time to process the request.
Please submit non-urgent requests on or after Tuesday, September 8th, 2026.
The Area Office on Aging of NWO, Inc. will re-open at 8:30 a.m. on Tuesday, September 8th,
2026.
