Hospital Discharge Planning, Nursing Home Admissions , and Social Work Departments
Office Closing -- Martin Luther King, Jr. Day
The Area Office on Aging of NW Ohio, Inc., PASSPORT Preadmission Unit will be closed on Monday, January 19, 2026, in observance of MLK Day.
There will be NO local or extended coverage provided on this day. (Jan. 19th,2026)
All requests submitted after midnight on Sunday, January 18, 2026, should be sent to the regular fax number (419) 382-0137. All submissions will be processed within OAC time frames.
Please try to submit requests for anticipated discharges on Friday, January 16, 2026, by 3 p.m.
For non-urgent requests, please submit them on Tuesday, January 20, 2026, when the Area Office on Aging will reopen at 8:00 a.m.
**Reminder -- HENS 3.0 is available 24/7 and is mandated for ALL PASRR REQUESTS**
